SAIL Waiver (for Physically Disabled Adults Ages 18-60

Types of Technology Reimbursed/Definition (i.e., Personal Emergency Response System (PERS), Medication Dispensing, Home Telehealth, Remote Monitoring, etc.)

PERS Service: This service will cover the monthly fee after the system has been installed. PERS is an electronic device which enables certain individuals at high risk of institutionalization to secure help in the event of an emergency.  The client may also wear a portable “help” button to allow for mobility.  The system is connected to the person’s phone and programmed to signal a response center once a “help” button is activated.

The response center is staffed by trained professionals.  PERS services are limited to those individuals who live alone, or who are alone for significant parts of the day, and have no regular caretaker for extended periods of time, and who would otherwise require extensive routine supervision. By providing immediate access to assistance, PERS serves to prevent institutionalization of those individuals.

PERS Installation: The same objective, provider experience, etc., for PERS (S6161-UB) will apply for this service. Price quotation from the vendor providing the service specifying the description of personal emergency requested. Set-up will be provided by individuals who are trained to install this device for specific consumers for whom services are being provided.

The maximum is a one-time installation charge. Once the recipient has had one installation, another one cannot be approved. (click for more program details)
