IL HCBS Waiver for Persons who are Elderly (for older individuals ages 65 - no max age and physically disabled individuals 60-64)

Types of Technology Reimbursed/Definition (i.e., Personal Emergency Response System (PERS), Medication Dispensing, Home Telehealth, Remote Monitoring, etc.)

PERS Service: Emergency home response service (EHRS) is defined as a 24-hour emergency communication link to assistance outside the participant's home for participants based on health and safety needs and mobility limitations. This service is provided by a two-way voice communication system consisting of a base unit and an activation device worn by the participant that will automatically link the participant to a professionally staffed support center. The support center assesses the situation and directs an appropriate response whenever this system is engaged by a participant. The purpose of providing EHRS is to improve the independence and safety of participants in their own homes in accordance with the authorized plan of care, and thereby help reduce the need for nursing home care. Services cover both initial one time installation and monthly rental costs.

IL Persons w/Disabilities (for physically disabled individuals ages 0-59)

Types of Technology Reimbursed/Definition (i.e., Personal Emergency Response System (PERS), Medication Dispensing, Home Telehealth, Remote Monitoring, etc.)

PERS Service: PERS is an electronic device that enables certain individuals to secure help in an emergency.  The individual may also wear a portable "help" button to allow for mobility.  The system is connected to the person's phone and programmed to signal a response center once a "help" button is activated.  Trained professionals staff the response center.  PERS services are limited to those individuals who live alone, or who are alone for significant parts of the day, and have no regular caregiver for extended periods of time, and who would otherwise require extensive routine supervision.  This service has two components: an initial installation fee and a monthly service fee. The amount, duration, and scope of services are based on the determination of need assessment conducted by the case manager and the service cost maximum determined by the DON score.
